    About the Role:

    The Licensed Practical Nurse (LPN) will deliver high-quality patient care and perform skilled technical procedures within the patient's home. Responsibilities include adhering to state nursing laws, following the care plan, and collaborating with the healthcare team without making changes to the plan of care. Additionally, they will educate the patient's family on caregiving and maintaining a safe environment.

    The LPN will also monitor and document the patient's response to treatment and any health changes on a daily basis for review by the supervising nurse or physician.


    License Requirements:

    • Active LPN license in the state of practice
    • Valid Driver's License, auto insurance, and access to a reliable vehicle or public transit
    • CPR Certification
