- Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical, financial, and psychosocial approach through the continuum of care.
- Performs concurrent clinical review to determine appropriateness of admission and continued need for care delivery in the acute care setting.
- Performs telephonic clinical reviews with claims insurance personnel to facilitate reimbursement for the delivery of patient care services.
- Assesses the patient for discharge planning needs and referrals to the interdisciplinary team.
- Supports collaboration and communication among the patient/family and entire health care team regarding the patient's plan of care, continued needs post-discharge, and facilitates those services required.
- Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization.
- Identifies quality and/or risk issues and reports these to the appropriate individual/department.
- Current licensure in the State of Colorado as a Registered Nurse, or current active multistate nursing licensure
- BLS certification through AHA or ARC
- Graduate of an accredited school of nursing; BSN preferred
- Three – Five (3-5) years recent clinical or case management experience in area of specialization desired

RN Case Manager
Thursday - Sunday Weekend Shift 8 and 10 hour shifts
Thursday/Friday will Float to Various Units
Saturday/Sunday will cover PCU and Medical Oncology Units
Weekend shifts will pay $8/hour differential
