    SUMMARY:
    The Pharmacist is required to provide pharmaceutical care based on physician orders. The Pharmacist prepares medications while monitoring for therapeutic incompatibilities. Supervises the preparation and dispensing of prescribed medications and supervises the activities of the support staff.


    RESPONSIBILITIES:

    • Reviews orders written by physicians and authorized prescribers and enters those into the PIS efficiently and accurately.
    • Independently works up patients to determine appropriate dosing of selected target drugs or when requested by the prescriber.
    • Appropriately monitors the effectiveness of prescribed medications by checking lab values and other patient parameters when dictated by protocol or by individual patient needs.
    • Follows up with nursing or prescribers when critical information elements are needed to process medication orders.
    • Participates in department mandated clinical activities such as IV/PO conversions, drug renal screens, dosing verifications for pediatrics patients etc.
    • Documents clinical interventions as required by department policy.
    • Seeks assistance from clinical pharmacist when appropriate.
    • Is familiar with and is able to independently initiate selected drug protocols when requested.
    • Effectively manages workflow during his/her shift, requests and/or provides assistance when appropriate.
    • Is knowledgeable of various department technologies and is able to assist new staff, nurses and others in their use. (eMAR, Omnicell, MedCarousel, NarcStation, barcode applications etc.)
    • Displays acceptable level of flexibility by accepting assignments in specialty areas such as Employee Pharmacy, Oncology or meeting/committee assignments when requested by the Director of Pharmacy.
    • Provides drug information to prescribers, nursing staff and patients when requested.
    • Supervises the support staff by assigning tasks as needed to optimize turnaround time and department operations.
    • Provides support to department financial objectives by managing drug line items, automatic drug interchanges, established inventory par levels and by accurately posting charges and credits.
    • Assists in the supervision and training of Pharmacy and Tech Students.
    • Keeps abreast of all pertinent federal, state and facility regulations, laws and policies as they presently exist and as they change or are modified.
    • Understands and adheres to compliance standards as they appear in the Corporate Compliance Policy, Code of Conduct, and Conflicts of Interest Policy.
    • Demonstrates age-specific skills as required by position.
    • Offers suggestions on ways to improve operations of department and reduce costs.
    • Attends all mandatory education programs.
    • Improves self-knowledge through voluntarily attending continuing education/certification classes.
    • Maintains required competency levels as identified in written exams, skills checklists, skills labs, annual safety and health requirements as well as service excellence education hours requirements.
    • Willing to cross-train in order to better assist co-workers and to provide maximum efficiency in department.
    • Volunteers/participates on hospital committees, functions, and department projects.
    • Manages resources effectively.
    • Reports equipment in need of repair in order to extend life of equipment.
    • Makes good use of time so as to not create needless overtime.

    EDUCATION:

    • Doctorate in Pharmacy in Pharmacy

    CREDENTIALS:

    • Pharmacist

    OTHER INFORMATION:

    • One (1) year's experience in a hospital pharmacy practice required.
    • Residency training may be accepted in lieu of experience requirement.
