    Case Manager

    Onsite locations: Lancaster, CA 93534 or Chatsworth, CA 91311

    • Completed Bachelor degree REQUIRED, in a related field
    • Minimum of 1 year of Social Work or Case Management experience

    Full-time | M – F | 8:30a – 5:30p

    Pay range: $25 per hour (Position is eligible for bonus structure, with great company benefits)

    Role requirements:

    • Provide comprehensive case management services to underserved individuals.
    • Conduct assessments to determine clients' needs and develop individualized service plans, schedule interdisciplinary meetings.
    • Connect clients with appropriate resources and services to address barriers.
    • Collaborate with community partners, shelters, and other agencies to facilitate access to resources and coordinate care.
    • Maintain accurate and up-to-date documentation of client interactions, progress, and service referrals.
    • Advocate on behalf of clients to ensure access to essential services and support their goals for housing stability and self-sufficiency.
    • Participate in team meetings, case conferences, and training sessions as required.

    What you bring

    • Minimum 1 year experience in Case Management or Social Work.
    • REQUIRED: Bachelor's degree in a related field.
    • Bilingual is a plus
    • Strong interpersonal skills and the ability to build rapport with diverse populations.
    • Excellent communication and problem-solving abilities.
    • Compassionate and non-judgmental approach to working with clients.
    Pay Details: $25.00 per hour
