- Provide direct patient care following the Nurse Practice Act in the Home Health and/or Hospice environment
- Collaborate with other disciplines on the case to ensure and deliver safe, timely, and appropriate quality patient care
- Create Plan of Care upon initial patient assessment and report to Care Team
- Provide medication management that includes IV Infusions, nursing interventions, pain management, specimen collection, teaching, and discharging upon completion of a plan of care
- Communicate and consult with Primary Care Physician as needed
- Perform other duties as assigned
- 2 years of Acute Care OR Home Health/Hospice Experience
- Monday - Friday 8:00AM - 5:00PM
- CA RN License
