+ Job summary +Corrections Community Rehab SpecialistThis position provides care coordination to individuals assigned to Community Corrections for supervision and are referred to Crawford County Mental Health for coordination and case management services.+ Qualifications +Graduation from high school or equivalency is required.A bachelor's degree from an accredited university in the various behavioral health fields such as clinical psychology, social work, counseling, nursing, rehabilitation or closely related degree is preferred.A valid driver's license is required.Pass pre-employment background checks.Previous experience in a community mental health center.Experience in helping professions and occupations is desired and helpful.+ Responsibilities +Collaborate with Community Corrections staff to address challenges that create barriers for receiving mental health services.Participate in case staffing to address risk/needs and to identify strategies to reduce risk.Assist Community Corrections staff to complete applications for benefits and assist with securing benefits.Serve as a point of contact for updates on joint clients.Assist in assessment of client needs.Assist in the development of treatment plans.Assist clients in accessing community-based resources including providing transportation for clients when necessary.Support access other health-related providers.Assist clients implementing tasks related treatment plan goals.Collaborate with community partners agencies assist following treatment plan goals assist securing appropriate community resources.Advocate empower clients ensure appropriate services provided necessary benefits secured.Work closely clinical medical staff provision medication services.Provide information support family members clients appropriately.Asst crisis management needed.Complete documentation services provided electronic medical record timely manner.Update demographic information applicable.Met department productivity standards Documentation Standards.+ Benefits Full-Time Benefits KPERs Covered
