Job summary
Amergis Staffing is partnered with a much respected Adult Day Health organization providing community-based care services that help elderly and disabled individuals remain safely and comfortably in their homes.As an LPN Case Manager, you will be responsible for assessing, planning, implementing, reevaluating and revising care for clients and caregivers.
You will update and maintain patient records, implement orders for medications and treatments and perform assessments.
You will work closely with the interdisciplinary Care Team in developing proactive action plans to best serve the client's needs.
