The Case Manager plays a crucial role in ensuring high-quality, patient-centered rehabilitative care to achieve optimal outcomes. Responsibilities include patient-specific care coordination, collaborating with the Rehabilitation team, and liaising with the Rehab Program Director and Facility Case Management Director.
