- Assesses clients' needs during intake and assessments as well as routine visits; identifies ways to provide resources and to support clients to address identified needs. Maintain a caseload of 25 clients.
- Works with clients on establishing client-driven goals based on needs assessment.
- Compiles an accurate and detailed client history.
- Works with clients from various backgrounds and experiences such as individuals who are unemployed, experiencing homelessness, previously incarcerated individuals, veteran individuals, individuals facing unplanned pregnancies, and/or individuals and families who are facing barriers that prohibit them from meeting their basic needs.
- Facilitate in-person and tele-meetings with clients.
- Provides clients with information and guidance on available resources and support programs; knowledgeable of available community resources and understands how to connect clients with needed services.
- Conducts follow-up sessions to ensure services are provided.
- Collaborates with internal and external partners to identify additional available community services.
- Bachelor degree in social work or a related human service field required (i.e. psychology). Master's degree preferred.
- At least 2 years case management experience.
- Must possess great communication (verbal and written) and organizational skills; problem solving and conflict resolution skills; knowledge of computers and software programs; ability to multi-task; and the ability to work with diverse populations.

- Possess great time management skills and can handle multiple demands and priorities.
- Must have the ability to interact positively with grantee, referral sources, other agencies, and community partners.
- Must also have the ability to comprehend, document, and converse in Standard English and demonstrated great sound judgment and reasoning skills. Spanish fluency is a plus.

Today, millions of lives have been transformed, in the communities we serve. We have office locations in the Kansas City metro-area, St. Joseph, Missouri, and two Food Pantries – one in south Kansas City and the other in Cameron, Missouri.
