Job summary
The Community Outreach Counselor is responsible for developing and implementing outreach strategies that increase awareness of Centerstone services among community members and potential clients. This role builds and maintains relationships with community stakeholders, attends events, conducts comprehensive outreach services including intervention education linkages to care the counselor meets with individuals and community partners in various settings such as hospitals and agencies ensures continuity of care screens assesses potential clients assists them in determining eligibility for Centerstone programs or other forms of support the counselor takes initiative coordinating outreach activities evaluating engagement outcomes under direct supervision.Responsibilities
- Knowledgeable on Centerstone programs services eligibility requirements availability speaking depth each program how begin services.
- Ensures continuity care by coordinating team families colleagues partners increase contacts organizations client referrals maintain relationships encourage use organization programs complete enrollment assessment eligible participants accurate records documentation conducting ongoing assessment evaluation needs adapt strategies better serve community implement inform public mission creates relationships successfully develop CCBHC clinics.
