- Have the ability to speak fluently in more than one language and would like to use your language skills to support families in the Greater Philadelphia region?
- Have the ability to travel within the Greater Philadelphia region for home visits and outreach?
- Possess strong interpersonal skills for effective collaboration with diverse groups?
- Have a desire to play a pivotal role in delivering home-based services that empower pregnant women and parenting families with children ages 0-3?
- Provide intensive case management services, including assessments and referrals, through in-person home visits in one of the following services areas: Berks County, Bucks County, Delaware County, Philadelphia County, Lehigh County or Montgomery County
- Flexibility is key - be ready to adapt your schedule to meet family needs.
- Assist families in identifying their health and social service goals.
- Support parent-child engagement and foster family well-being through reflective strategies.
- Collaborate with community resources to ensure comprehensive support for families.
- Conduct health promotion activities and deliver evidence-based parenting education.
- Associate's degree in Social Work, Public Health, or related field of study such as: Family Studies, Women's/Gender Studies, Human Services and Education.
- Experience with community health issues.
- Valid driver's license with driving experience and a clean driving record.
