    Congestive Heart Failure Nurse Practitioner Opportunity

    Join our team at St. Vincent's Medical Center, a nationally recognized cardiac program in Bridgeport, CT.

    • Autonomously manage a patient panel of recently discharged and chronic acute exacerbation patients in an outpatient ambulatory clinic setting.
    • Remote monitoring of heart failure patients, managing LVAD patients, collaborating with cardiologists, and oversight and referring out for comorbidities.
