Job summary

A Physical Therapist Assistant provides therapy services in a home care setting under the direction of a Physical Therapist.

Responsibilities

Provide therapy services planned, delegated and supervised by the Physical Therapist consistent with the patient's approved Plan of Treatment
Observe patients during treatment, compiles data and report patient's progress to the Physical Therapist
Maintain contact with patients, Physical Therapist, RN Case Managers and other members of healthcare team in assuring effective care coordination

Requirements

Degree from an accredited Physical Therapist Assistant Program (approved by APTA)
