The Social Worker Designee provides support and advocacy for residents and their families within a healthcare or long-term care setting.This role ensures compliance with state and federal regulations, promotes resident rights, and assists in addressing psychosocial needs to enhance quality of life.
Responsibilities
- Serve as liaison between residents,families,and the care team to address concerns and ensure satisfaction. ,
- Assist with admissions transfers,and discharges ensuring smooth transitions proper documentation. ,
- Support residents coping with emotional social environmental challenges. ,
- Maintain accurate records of resident interactions care plans in compliance with regulatory standards.
