- Provides an admission assessment for new residents:
- Appropriately updates the assessment and physician orders
- Delegates activities to the Home Health Aides and Licensed Practical Nurses as appropriate according to nursing judgment and state regulations
- Provides supervisory visits according to nursing judgment and state assisted living licensure regulations
- Reviews and updates the service plan to assure that it is current and accurate
- Completes discharges and transfers for residents when needed
- Establishes and implements medication management for residents based on assessed data and according to agency policy and state statute
- Documents as indicated in clinical record
- Responds to clinical problems as identified by the staff in the facility
- Communicates with families, resident representative(s), case managers and other health care providers regarding the residents when indicated
- Provides direct nursing care and interventions for residents with episodes of potentially life-threatening instability, those with complex equipment needs and those requiring complex nursing care
- Documents, as indicated, in the clinical record
- Is aware of and uses good safety practices, including but not limited to:
- Body mechanics
- Falls prevention
- Reporting of defective equipment or hazards
- Wiping spills or wet floors immediately
- Using Standard Precautions and appropriate infection control measures
- Knowledge of emergency procedures
- Reporting incidents/accidents to the Clinical Nurse Supervisor or Licensed Assisted Living Director on the day of occurrence
- Participates in care conferences with other members of the health care team
- Maintains absolute confidentiality of all information pertaining to residents, families, and employees
- Promotes a safe and comfortable environment for the resident and family
- Demonstrates excellent written and verbal communication skills
- Recognizes and manages own stresses, which may affect work performance
- Participates in team meetings
- Participates in in-service educational programs, as required by state regulations
- Performs additional duties as assigned by the Clinical Nurse Supervisor or Assisted Living Director
- Adheres to all agency policies
- Currently licensed Registered Nurse (RN) in state of employment
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Valid drivers license
- First aid/CPR certification required

Experience in home care or assisted living preferred
Supervisory experience preferred

Able to function independently
Good health status and emotional stability
Demonstrate a strong commitment to service excellence
