    Job DescriptionREPORTS TO: Clinical Nurse Supervisor

    PRIMARY RESPONSIBILITY: The Registered Nurse is responsible for providing clinical nursing services to assisted living residents.

    JOB RESPONSIBILITIES:

    1. Provides an admission assessment for new residents:
    Completes all admission paperwork in a timely manner
    Prepares a service plan for each resident with input from the resident and/or designated representative
    Orients direct care staff to the service plan
    Obtains physician orders, as appropriate
    Obtains needed equipment, supplies and medications for residents
    1. Appropriately updates the assessment and physician orders
    2. Delegates activities to the Home Health Aides and Licensed Practical Nurses as appropriate according to nursing judgment and state regulations
    3. Provides supervisory visits according to nursing judgment and state assisted living licensure regulations
    4. Reviews and updates the service plan to assure that it is current and accurate
    5. Completes discharges and transfers for residents when needed
    6. Establishes and implements medication management for residents based on assessed data and according to agency policy and state statute
    7. Documents as indicated in clinical record
    8. Responds to clinical problems as identified by the staff in the facility
    9. Communicates with families, resident representative(s), case managers and other health care providers regarding the residents when indicated
    10. Provides direct nursing care and interventions for residents with episodes of potentially life-threatening instability, those with complex equipment needs and those requiring complex nursing care
    11. Documents, as indicated, in the clinical record
    12. Is aware of and uses good safety practices, including but not limited to:
      1. Body mechanics
      1. Falls prevention
      1. Reporting of defective equipment or hazards
      1. Wiping spills or wet floors immediately
      1. Using Standard Precautions and appropriate infection control measures
      1. Knowledge of emergency procedures
      1. Reporting incidents/accidents to the Clinical Nurse Supervisor or Licensed Assisted Living Director on the day of occurrence
    13. Participates in care conferences with other members of the health care team
    14. Maintains absolute confidentiality of all information pertaining to residents, families, and employees

    2. Demonstrates excellent written and verbal communication skills
    3. Recognizes and manages own stresses, which may affect work performance
    4. Participates in team meetings
    5. Participates in in-service educational programs, as required by state regulations
    6. Performs additional duties as assigned by the Clinical Nurse Supervisor or Assisted Living Director
    7. Adheres to all agency policies


    EXPERIENCE/JOB SKILLS REQUIREMENTS

    Graduate of accredited nursing program and licensed as a registered nurse in the State of Minnesota
    Experience in home care or assisted living preferred
    Supervisory experience preferred
    Must be able to read, write, comprehend and carry out directions and instructions
    Able to function independently
    Good health status and emotional stability
    Demonstrate a strong commitment to service excellence

    WORKING CONDITION REQUIREMENTS

    PHYSICAL DEMANDS: (Occasionally means 1-2 hours of an 8 hour workday, frequently means 2.5 to 5.5 hours of an 8 hour workday and continually means 6 to 8 hours of an 8 hour workday)

    Qualifications
    • Currently licensed Registered Nurse (RN) in state of employment
    • Previous experience as a Registered Nurse is preferred
    • Excellent interpersonal and client care skills
    • Valid drivers license
    • First aid/CPR certification required
