    Job Summary:

    Evaluate member assessments and develop and coordinate social work care plans to meet the needs of members and families enrolled in the Health Home Program.

    Provide crisis intervention and assist families in understanding the implications and complexities of a member's medical condition and care needs.

    Support members in the determination of community resource needs and identification and attainment of available resources in the community. Communicate with care team and intervene on immediate social and behavioral needs. Perform scheduled outreach for assigned members and consult with the care team on their daily outreach. Intervene for high utilization of ED visits for those with MSW needs. Schedule annual review with members. Communicate and document care gaps. Work with outside resources for carve out hand offs for behavioral health needs.


    Compensation:
    The initial pay range for this position upon commencement of employment is projected to fall between $33.00and $40.00.

    However, the offered base pay may be subject to adjustments based on various individualized factors, such as the candidate's relevant knowledge, skills, and experience.

    RequirementsEducation and Experience:
    Master's degreeTwo or more years of experience working in a health care setting coordinating social services for members and their familiesExperience working with homeless population preferredMust possess a valid California driver's license, clean driving record and current insuranceBilingual English and Spanish preferredJob knowledge skills and abilities:Ability to demonstrate respect for the needs of members, support givers, team members, and others and demonstrate excellent customer serviceWillingness to work collaboratively as part of a team all levels of professionals to achieve goals and identify and remove barriers to member healthAbility to demonstrate a sensitivity to member's social, cultural, language, physical and financial differencesAbility to work with members and influence behavior through negotiation of care goals and support of member self-managementDemonstrated skills in crisis interventionStrong problem-solving skills and ability to identify issues and solutions in administrative and other relevant areasAbility to set and re-set priorities based on changes in member situation and needsAbility to work independently, organize and prioritize multiple priorities throughout the dayStrong attention to detail and ability to be accurate, thorough and persistent in solving problems and following tasks through to completionExcellent verbal and written communication skills; able to read, write, and speak articulately and communicate effectively with all levels of the organization and member spectrumDemonstrated ability to create professional documents and use proper grammar and punctuationAbility to be handle all situations in a professional manner and adapt to a changing environmentAbility to understand and communicate complex health and benefit informationProficient in the use of common office technologyReliable in attendance and work scheduleAdherent to business dress code and good groomingAbility to always maintain strict confidentialityPossible travel from office and throughout the community
