    Client seeking LVN/LPN/RN for per diem assignment in Fort Wayne, IN on April 19, May 9, 10, 13, 24, June 20, 21, July 5, 8.

    Hours are Monday/Friday, 8:00am - 3:00pm; Thursday, 7:30am - 6:00pm. Provider will see employees and their families, ages 2+, for primary care and acute care. Candidates must have 2+ years of experience to be considered for this assignment.
