Shift: Day

Start Date: 05/27

• Required to Work Weekends, Potentially 3 Weekend Shifts Per Contract. Rotating Shifts Like Evening And Holidays.
• Floating Is Required Within the Hospital.
• 2 Years Case Management Experience Required.
• Case Manager Nurses Ensure Patients Receive Appropriate Quality Care That Is Cost Effective. Works Directly With Patients And Their Family Members To Conduct Assessments & Analyzes Data To Determine Patient's Needs.
• Collects Data While Working Closely With The Members Of the Patient's Health Care Team.
• Case Load Roughly 20-30 Patients Per Day.
• BLS Required.
• Valid AZ or compact RN License required.
