    Description

    Clinical Pharmacist Specialist- Antimicrobial Stewardship

    • 2 years of relevant experience in hospital pharmacy or an ASHP Pharmacy Practice Residency is preferred
    • Provides leadership for all Pharmacy clinical activities within assigned area, promoting team efforts to achieve departmental/corporate objectives
    • Supports staff development and encourages professional growth, serving as a role model of clinical excellence
    • Assists in controlling and maintaining pharmacy drug budget, ensuring appropriate medication formulary use
    • Develops and oversees data collection for drug utilization review, quality improvement monitors, and other processes

    Essential Functions

    • Provides quality patient-specific pharmaceutical care and documents interventions
    • Participates in direct patient care, providing drug information and recommendations
    • Assists in organizational quality improvement activities and formulary decisions
    • Participates in patient education programs and medication therapy review
    • Supports clinical development, training, and education of healthcare practitioners
    • Participates in medication error prevention and organizational subcommittees
    • Performs operational and clinical duties when necessary

    Qualifications

    • Required Bachelor's Degree in pharmacy
    • Residency training is preferred
    • 3 years of relevant experience as a clinical pharmacist or pharmacy residency/fellowship
    • LIC-Clinical Pharmacist and LIC-Pharmacist Controlled Substance required
    • Immunization Certification and Basic Life Support certification required
