- Design personalized treatment plans based on assessment outcomes and discuss them with patients.
- Perform psychosocial assessments, provide counseling and referrals for individuals, couples, families, or groups, and keep track of progress notes and billing procedures.
- Establish communication with referral sources, providers, and caregivers to ensure seamless patient care.
- Evaluate program effectiveness and contribute to the improvement of existing and new initiatives.
- Conduct utilization reviews as needed.
- Collaborate with the Primary Care Provider (PCP) to monitor patient progress and evolving needs.
- Undertake any other assigned duties as required.
- Licensed Clinical Social Worker (LCSW) through North Dakota Board of Social Work Examiners before the Start Date.
- Licensed Independent Clinical Social Worker licensed by the Minnesota Board of Social Work within 12 Months of Start Date.
- Licensed Professional Clinical Counselor (LPCC) accredited by North Dakota Board of Counselor Examiners prior to Start Date and by Minnesota Board of Behavioral Health and Therapy within 12 Months of Start Date.
- Basic Life Support (BLS) certification from the American Heart Association (AHA) within 90 Days of Start Date.
- Can be licensed as LCSW or LPCC.
- Credentialing will be processed through Altru Health System's system.
