Job Summary
Join our PCHHC Family as a Home Health & Hospice Licensed Practical Nurse (LPN), where you will play a crucial role ensuring our patients receive exceptional care. Our LPN's are responsible for verifying and executing the plan of care as prescribed by physicians and under the guidance of a Registered Nurse. This includes providing essential nursing treatments, promptly addressing patient pain, assisting with activities of daily living, and educating both patients and caregivers on self-care techniques. They also handle medication administration, ensure the availability of necessary supplies, maintain accurate patient records, and consistently liaise with supervising RNs or physicians on patient assessments and any changes in conditions.
