- Provide health education according to the teaching plan established by the RN.
- Provide routine LPN nursing care as outlined in the nursing care plan and within the scope of practice for an LPN.
- Observe signs and symptoms , and report patient's reactions to treatments including drugs and changes in the patient's physical or emotional condition , to the physician and RN Case Manager .
- Document any services rendered to the patient, changes noted in the patient's condition , and/or family and home situations.
- Document communication with RN C ase M anager and physician.
- Other duties as assigned.
- Associate's degree required .
- Diploma as a Licensed Practical Nurse from an accredited program.
- Current, valid LPN license issued from NYS Department of Education.
- Experience in home health care preferred.
- Community health experience preferred .
- Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
