Job summary
The Habilitation Specialist is responsible for developing, documenting, and implementing Residential Habilitation Services for people living in state-operated community residences.
Under supervision of a Habilitation Specialist II they are responsible for developing service plans that include discussions with individuals advocates Care Coordinators and residential providers.
They provide training technical assistance to residential staff on implementation of habitation plans assist individuals in meeting valued outcomes facilitate completions of assessments prepare distribute meeting minutes coordinate regular meetings with clinical team members.
