    Minimum Qualifications:

    A qualifying Bachelor's Degree in Human Services and 1 year clinical/or direct care experience or a qualifying Master's Degree in Human Services.

    QIDP standards preferred.


    QIDP STANDARDS ARE:

    • The Department of Developmental Services (DDS) adopted the federal definition for Qualified Intellectual Disabilities Professional (QIDP) in all ICF/DD, ICF/DD-H, ICF/DD-N and DD-CNC facilities when the Federal ICF/IID regulations became effective in October 1988. The federal criteria outdates the state regulations specified in the California Code of Regulations, Title 22.
    • QIDP is a staff who is responsible for integrating, coordinating and monitoring each client's active treatment and who possesses: (a) At least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities.
    * (b) Is either a physician and surgeon, or a osteopathy physician and surgeon, a registered nurse, or a human services professional possessing at least a bachelor's degree in a human services field, including but not limited to sociology, special education, rehabilitation counseling or psychology

    Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:

    • Coordinate, develop and provide Community Habilitation-Residential (CH-R) services for Individuals in an IRA setting.
    • Provide Recreation and Community inclusion opportunities to Individuals in an IRA setting.
    • Develop and modify Community Habilitation-Residential Plans; Safeguard Summary Plans; Community Inclusion Plans; Money management assessments and Personal Expenditure Plans as needed.
    • Coordinate with Clinical staff to ensure all individual needs are met and programs are developed/modified as appropriate.
    • Provide training and hands on assistance in the proper delivery of daily services.
    • Provide oversight of inclusion experiences by participating in community inclusion activities for individuals.
    • Provide training and oversight in the proper documentation of Medicaid billing documents; Community Habilitation-Residential Monthly notes; and EHR notes in relation to the implementation of services.
    • Develop and maintain a good working relationship with Medicaid Care Coordinators and staff that serve individuals.
    • Attend Life plan reviews for all individuals, work collaboratively with Care Coordinators and update Community Habilitation-Residential Plans and Safeguard Summary Plans that support the Life plan.
    • Develop and maintain a good working relationship with IRA managers and House staff to ensure the implementation of Community Habilitation-Residential Plans and staff resources are available for the needs of the individuals.
    • Effectively and consistently communicate with the IRA managers, house staff and other team members.
    • Be responsible for the content and organization for the individual program charts and house filing system in relation to Habilitation services.
    • Participate in annual chart audits.
    • Assist with other Habilitation service needs as warranted.
    • Function as a liaison for individuals in the community.
    • Attend and participate in all mandatory trainings.

    NY HELPS This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment.

    This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

    If you are hired under the HELPS Program, you will be first be appointed as a temporary employee.

    In May 2024, if you are performing successfully in the position, your appointment will be changed from temporary to permanent non-competitive.

    Once you are in permanent status, your official probationary period will begin and you may take any promotion exam for which you are qualified.

    At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status.

    You will not have to take an exam to gain permanent competitive status.


    • The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.
    • Preference will be given to OPWDD employees impacted by closures. If you are being impacted by closure, please indicate this on your resume.
    • All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.
    • If you are employed by state government you may be able to receive loan forgiveness under the Public Service Loan Forgiveness Program (PSLF). The PSLF Program forgives the remaining balance on Direct Loans after 120 qualifying monthly payments have been made under a qualifying repayment plan while working full-time for a qualifying employer.
