About The Crocker Care Campus & Harm Reduction Model
The Crocker Care Campus is a radical, patient-centered space designed as a welcoming drop-in site for people who use drugs and those experiencing homelessness. It provides a safe accessible environment focused on hospitality community and comprehensive care Unlike traditional clinics the Crocker operates with flexibility prioritizing low-barrier engagement and participant autonomy over rigid structures Our approach acknowledges the complex medical psychosocial histories of our participants including individuals who use drugs engage in sex work have been incarcerated or faced structural racism poverty and trauma At Crocker we are committed to nonjudgmental compassionate care rooted in social justice Principal Responsibilities Perform routine patient care procedures technical functions within the LVN scope of practice Conduct basic assessments collect data document patient information following JWCH policies and LVN practice standards Contribute individualized treatment plans interventions Support harm reduction strategies safer use practices overdose prevention risk reduction counseling Provide wound care medication reconciliation/management phlebotomy for challenging venous access patients education Assist patients with routine hygiene needs e.g changing clothes wound cleaning Navigate patients through addiction medicine services ensuring follow-up retention in care Collaborate interdisciplinary team LCSWs case managers harm reduction specialists primary providers behavioral health clinicians Assist administering vaccinations monitoring vitals performing other standard nursing duties Maintain clean well-stocked work environment clinical non-traditional settings Ensure documentation accurate timely HIPAA-compliant Participate staff training program evaluation quality improvement initiatives Performs other duties assigned Requirements Current valid California LVN License Current BLS certification American Heart Association At least one year experience ambulatory care community health setting Familiarity willingness learn harm reduction drug user health addiction medicine Commitment patient-centered unconditional positive regard particularly individuals experiencing homelessness drug use sex work incarceration Understanding unique healthcare needs populations served chronic acute sequelae homelessness drug use incarceration Ability work non-traditional clinical setting e.g outdoor parklets drop-in spaces Demonstrated ability independently collaboratively interdisciplinary team Strong communication documentation skills Bilingual EnglishSpanish preferred Flexibility both morning evening shifts needed Preferred Experience Skills Experience working marginalized populations community health harm reduction setting Proficiency evidence-based behavioral interventions motivational interviewing trauma-informed relapse prevention Experience nurse-only visits wound medication management phlebotomy difficult venous access All JWCH Wesley Health Centers workforce members recommended fully vaccinated boosted COVID-19 Become part team your matters Apply today help change lives one patient time JWCH Institute Inc + Wesley Health Centers Equal Opportunity Fair Chance Employer Shift Weekend Availability Preferred Friday Monday
