The Home Care Coordinator LPN orchestrates the delivery of health care services for PACE participants in all care settings according to their plan of care.
QUALIFICATIONS:

Education:
Graduate of a school of professional nursing. Current North Carolina Licensed Practical Nurse (LPN) License, Cardiopulmonary Resuscitation (CPR) certification required.
Experience:
Minimum of two years of experience providing skilled nursing care.
One year of community health nursing/home care agency experience preferred. Experience in performing comprehensive geriatric assessments preferred.
