Conducts initial assessment of prospective residents including off site visits performs wellness assessments and develops individualized care plans based on the needs and preferences of each resident delivers health promotion services offers disease prevention counseling and provides self care education to residents engages with residents family members in a compassionate impactful manner creating a lasting positive experience fosters communication by regularly engaging with physicians pertinent healthcare providers family members hires trains schedules leads clinical team providing exceptional care experiences coordinates communities infection control program while maintaining state regulatory compliance develops oversees medication management program contributes toward quality assurance performance improvement initiatives complies all rules regulations regarding confidentiality privacy resident information.
