- Ensures completion of individual, family, and group assessments.
- Provide short term crisis intervention, as needed.
- Assist patients and families with community resources for post-hospital needs.

Job DescriptionSalary:Responsible for the provision of care to meet the physical, psychological, and social needs of all patients. This position ensures that each patient is included in healthcare decision-making, as well as works toward creating and sustaining an environment that humanizes and individualizes the care for each patient. This position serves as an advisor for patients and families in decisions about care for immediate needs, as well as aftercare. Provides range of clinical services to include, but not limited to, assessments of individual psychosocial needs, financial limitations, acute treatment and crisis prevention, as well as post-hospital care.
Supervisory Responsibility:

Position Qualifications:
Master's degree required
LSW in Ohio required, LISW preferred
Four years of experience, minimal
In-depth knowledge of psychotherapy methods and available community resources
Understanding of resiliency and recovery in mental illness and substance abuse, and treatment approaches and techniques.
