- Currently licensed as a Physical Therapist (PT) in this state and in good standing with the Board in which he/she will practice.
- Knowledge of principles, concepts and methodology of physical therapy.
- Ability to work well with an interdisciplinary team.
- Ability to develop and maintain rapport with the Patient and family.
- Excellent written and verbal communication skills.
- Demonstrate problem solving and decision making abilities.
- At least one year or more work experience in occupational therapy in a multidisciplinary setting is preferred.

POSITION SUMMARY
Total Compensation Package - $90-95,000
Responsible to the Director of Professional Services, Clinical Manager to provide medically prescribed, goal oriented therapy in accordance with the Physician Plan of Care, Company Policies and professional Standards of Practice.
JOB RESPONSIBILITIES
Participates in the development of patient physical therapy plans of care. Makes visits to the patients for assessments and evaluations and communicates with the physician as needed. Implements the Plan of Care.
Documents the care and submits electronic documentation in an accurate and timely manner to update and maintain the patient medical records.
Provides education and instruction to team members, patients and their families.Participates in regularly scheduled interdisciplinary team meetings to coordinate the care of the patient and agency and family, exchange information and problem solve, and receive staff support and education.
Applies the policies and procedures of the agency and the rules and regulations of Federal and State Regulatory agencies and Accreditation Commission for Health Care in providing care.
Coordinates and develops patient/family plan of care and ensures the care plan is current, up to date and reviewed at every team meeting.
Assesses & evaluates patient's level of care, needs and requirements. Completes electronic paperwork in an accurate and complete manner and adheres to agency policy for timely submission.REQUIRED EXPERIENCE, EDUCATION, SKILLS & ABILITIES
