Alaska Native Tribal Health Consortium has a hiring preference for qualified Alaska Native and American Indian applicants pursuant to P.L Indian Self Determination Act.
Under the general supervision of the Chief Nurse Executive, or designee.
Manages and oversees the functions and staff for the Continuum of Care Management Program at Alaska Native Medical Center (ANMC).
Assures integrated Continuum of Care Services that provide services and materials to patients and their primary care providers. Facilitates access to appropriate levels of care.Responsibilities
Manages Continuum of Care Management program, and supervises assigned staff. Provides direct services for high risk patients and program development to serve high risk populations.
Develops processes, procedures and programs to assist patients from all over the state presenting to attain appropriate health care.
Plans, develops and implements Continuum of Care Management Programs and activities are for internal and external customers and resolves arising issues.
Oversees and maintains department's improvement processes and activities. Facilitates patient flow through acute care to meet individual patient needs and to appropriately use acute care resources. Monitors health care and utilization trends to keep programs up to date and to give feedback to Senior Management. Ensures quality control on related health care systems.Explores other models of health care, analyzing the potential for success at ANMC. Assures an overall integrated Continuum of Care Management Services and discharge materials to patients and their primary care providers.
Identifies trends and issues which may indicate possible need for special projects. Researches scope of project, considers impact upon services and need to pursue further. Assigns resources and monitors progress of Continuum of Care staff. Serves as organizational expert in complex community discharges and safety of discharge issues.
Updates policies related to psychosocial or continuum of care issues.
Identifies need for new such policies and writes such policies for consideration by senior management, new policies psychosocial and patient environment safety of discharge and continuum of care issues as outlaid by senior management.
Keeps abreast of issues, develops and maintains CE program to meet licensure and information update needs of assigned staff. Gathers internal or external issues/inputs, opinions, analyzes solutions and makes decisions as required.Ensures Joint commission and other regulatory standards are met and documented. Promotes an environment that is customer focused and in alignment with ANMC mission, vision and values. Oversees clinical social work services as required.
Serves as staff to the ANMC Medical Ethics Committee. Writes and updates policies for consideration by the Ethics Committee and ANMC Senior Management.
Identifies and manages special projects. Researches scope of project, considers relevancy and relative importance. Assigns resources, and monitors progress of Continuum of Care Staff. C onsiders impact upon ANMC and need to pursue further.
Prepares and processes special and recurring reports and administrative records. Develops and monitors budgets.
Performs other duties as assigned.

MINIMUM EDUCATION QUALIFICATION
A Master's degree in Social Work or Master's in Nursing. Progressively responsible work experience may be substituted for nursing education on a year by year basis.
Minimum Experience Qualification
Non-supervisory – None
AND
Supervisory – Seven (7) years of supervisory experience.
MINIMUM CERTIFICATION QUALIFICATION
Current registered nurse license in the State of Alaska or Licensed Clinical Social Worker.
Preferred Experience Qualification
Experience in the Alaska Tribal Health System.
Additional Requirements
Depending on the needs of the organization , some incumbents in this job class may be required to obtain additional certifications or training in one or more specialty areas.
