- LVN Licensure for the State of Texas, or a compact Licensure.
- 2-3 years' experience in clinical or community health settings.
- Previous Care Coordination and/or Case Management experience preferred.
- Demonstrates evidence of essential communication, education, and some counseling skills.
- Proficient in communication technologies (email, cell phone, etc.).
- Effective organizational skills, demonstrates ability to maintain accurate notes and records.
- Previous experience with health IT systems and data reports preferred.
- Previous experience with mobilizing community resources, navigating through the healthcare continuum and working with disparate populations preferred.
- Ability to speak a relevant second language preferred.

Summary: The Care Coordinator Assistant (CCA) partners with members of the care team to coordinates patient care, navigate patients through the healthcare system to facilitate services to individuals, families and/or their communities through effective partnerships with patients their caregivers and their physician. Promotes a shared goal model that is appropriately scheduled for their clinical needs and connected to clinical, social, and community resources. The person in this role works collaborate with other across the healthcare team to ensure the patients specific healthcare needs are appropriately coordinated and medically managed.
