    Position Summary


    Reporting to the Director, Home-Based and Post-Acute Care Operations, this position will be responsible for identifying, screening and enrolling patients in the Hospital Care at Home program, an innovative care model that allows us to deliver inpatient level care to patients in their home.

    The role will work with administrators, physician leadership, nursing, case management and social work, and in-home clinical care teams.

    The Navigator is actively involved with patient recruitment, ensuring patient eligibility and providing consent information to eligible patients in the Emergency Departments, Inpatient units and Outpatient Ambulatory Centers.

    The role will also assist with the coordination of the patient transfer from the hospital to their home in a timely manner.

    In addition to supporting day-to-day operations, the Navigator will assist with programmatic administration to ensure the program remains compliant with hospital policies and procedures as well as regulatory requirements.

    The Navigator will have the ability to organize and prioritize work in a fast-paced, matrix environment, work independently, be self-directed and adapt to unpredictable circumstances.

    This position will primarily be on-site, with travel required between various hospitals, ambulatory locations and administrative buildings.

    Must be available to work flexible hours including weekends and holidays.

    Minimum Qualifications

    Bachelor of Science in Nursing or equivalent combination of education and experience.

    Unincumbered licensure in the state of Ohio as a Licensed Registered Nurse.

    Minimum of 2-5 years clinical experience in the acute care setting, preferably in a medical surgical environment.

    Clinical experience in discharge planning or case management preferred.

    Current unrestricted driver's license.

    Background in business administration and/or IT is preferred.

    Ability to work independently, be self-directed and adapt to unpredictable circumstances.

    Effective interpersonal, verbal and written communication skills are essential.

    Computer savvy and effectively utilizes technology to gather, report and analyze clinical data.

    Proficient with Microsoft Suite, including Outlook, Word, Excel, PowerPoint, and Teams, and EPIC EMR.
