Description
Position DetailsProvide quality client centered case management services that facilitate recovery and wellness for individuals who identify as members of the LGBTQIA+ community. Services are provided in both Kent and Ottawa county. The Case Manager will work with clients, families and support networks to implement treatment plans. Evaluate clients progress periodically and make adjustments as needed to improve outcomes. Positive outcomes are achieved through purposeful and respectful relationships rooted in trauma informed care, hope and evidence-based research. The Case Manager will have a passion to strengthen and support community engagement activities to raise awareness and acceptance. An openness to embrace and explore bias, equity issues, successes and challenges from a place of understanding and curiosity is needed to succeed and develop in the position /organization
