    Job Summary:

    The main role of the Medical Social Worker II is to support patients and families in managing the social and emotional challenges related to the patient's illness. Working under the guidance of the Social Work Manager/LCSW, the role involves delivering age-appropriate social work care to patients and their caregivers in line with agency policies and state/federal regulations. As a key member of the healthcare team, the Medical Social Worker II conducts assessments, coordinates care, develops treatment plans, offers information and referrals to community resources, and provides other social work services to address the complex needs of patients and families in hospital and clinic settings.


    Key Responsibilities:
    • Provides various interventions including psychodynamic interventions, crisis intervention, grief/bereavement counseling, and stress reduction in individual, family, and group settings.
    • Develops and implements individualized treatment plans to assist patients and families in coping with social, emotional, financial, and environmental factors affected by illness.
    • Conducts psychosocial assessments, collaborates with patients to identify needs, and creates treatment plans based on mutual agreement.
    • Explores care options, including internal and external resources, to help patients and families develop short- and long-term care plans.
    • Collaborates with different disciplines to assess and plan services for patients using biopsychosocial information.
    • Advocates for patients to access appropriate services within the organization and the community.
    • Aids in care and discharge planning for patients and families.
    • Reviews and prioritizes referrals, maintains documentation, and completes required documents promptly.
    • Assists in placing Kaiser members in nursing facilities and coordinates communication between various parties.
    • Identifies suitable care levels and facilities for referred patients, where necessary.
    • Links patients and families with community resources, facilitates information exchange, and determines benefits application as needed.
    • Participates in Utilization Management and Quality Assurance activities.
    • Coordinates with regional offices, clinics, hospitals, and field staff, triages phone calls, and ensures complete clinical information.
    • Performs other relevant duties as assigned.

    Grade 10

    Basic Qualifications:
    Experience
    • Step I: 0 - 2 years of social work experience.
    • Step II: 2 - 4 years of social work experience within the last five (5) years.
    • Step III: 4 or more years of social work experience within the last ten (10) years.
    Education
    • Masters in Social Work accredited by the Council of Social Work Education.
    License, Certification, Registration
    • N/A

    Additional Requirements:
    • Ability to work effectively in a multidisciplinary team setting.
    • Strong psychosocial assessment skills.
    • Knowledge of chronic and acute diseases and their impact on patient and family functioning.
    • Excellent communication skills, both oral and written.
    • Proficiency in using clinical information systems.
    • Capacity to handle a demanding workload and multiple tasks efficiently.
    • Independent yet collaborative team player.
    • Detail-oriented and knowledgeable about benefits and care coordination.
    • Willingness to work in a team-oriented environment.

    Preferred Qualifications:
    • At least one (1) year of post-MSW experience in a healthcare setting, with preference for LCSW certification.
