    JOB DESCRIPTION:

    This role involves providing personal care and specific health-related services aimed at preserving the physical and emotional well-being of clients residing at RM Management. Responsibilities include assisting clients with daily tasks outlined in the Service Agreement and Medication Administration Record (MAR), as well as other delegated services by the RN.

    REQUIREMENTS:

    • Preferably a High School education with CNA/ NAR or HHA Certificate.
    • Experience in working with elderly individuals.

    SKILLS AND ABILITIES:

    The ideal candidate must:

    • Demonstrate a compassionate approach towards working with seniors.
    • Effectively prioritize and organize tasks.
    • Have excellent communication skills with clients, families, staff, and other stakeholders.
    • Possess basic literacy skills to comprehend instructions, correspondence, and medical terms in English.
    • Be punctual with a solid attendance record and display strong interpersonal skills.
    • Know how to use and handle a cell phone responsibly while on duty.

    KEY RESPONSIBILITIES:

    Medication Administration:

    • Administer medications and reminders to clients following the MAR and RN Supervisor's instructions.
    • Adhere to medication administration policies, including PRN standing orders.
    • Follow protocols for narcotic administration and documentation.
    • Manage medication reorders and supplies regularly.
    • Understand and practice standard infection control precautions.
    • Perform other medication-related duties delegated by the RN Supervisor.

    Personal Care Assistance:

    • Support clients with daily living activities stated in their service agreement.
    • Assist with personal grooming, housekeeping, and routine procedures under RN Supervisor's guidance.
    • Aid clients with laundry and maintaining a hygienic environment.
    • Encourage client independence by offering choices and fostering self-help skills.

    Documentation and Reporting:

    • Record any changes in clients' conditions in charts and communication logs.
    • Complete incident reports as per guidelines.
    • Report medication changes and PRN administrations to the RN Supervisor.
    • Follow home care task procedures and documentation protocols.
    • Recognize and report any abnormal signs or symptoms promptly.
    • Document tasks performed with clients in the service log during each shift.

    Compliance and Paperwork:

    • File papers in clients' charts when necessary.
    • Monthly signing of Medication Administration Records (MAR's).
    • Review and sign clients' care plans and communicate changes effectively.
    • Stay updated with state regulations and compliance standards.

    Activity Programming:

    • Engage and support clients in scheduled activity programs.
    • Lead assigned activity programs and incorporate client suggestions.
    • Socialize with clients to enhance their participation and experience.

    Team Collaboration:

    • Maintain positive relationships with colleagues from various departments.
    • Provide backup support to ensure continuous 24-hour staffing.
    • Update the team and families on any changes in clients' care needs.
    • Participate actively in team meetings and contribute constructively.
    • Handle emergency situations calmly and respond to safety hazards promptly.
    • Assist kitchen staff as needed during meal times.
    • Provide training to new associates and maintain a professional demeanor.

    Additional Responsibilities:
