    Your opportunity


    We are looking for a Patient Outreach Coordinator that will communicate directly with patients and provider offices, help schedule appointments for patients, review medical records, and gather any relevant information to support the care team ahead of a patient's appointment.

    The Patient Outreach Coordinator is expected to deliver high quality customer service and will facilitate timely access to care.

    This is presently a hybrid model role, with an expectation that you will work in the centralized office, visit provider practices as directed, and at times, work from home.

    This position reports to the Patient Outreach Supervisor.

    Duties and Responsiblites


    • Serve as an essential link between patients and all other care providers.
    • Assist with the increase of performance in quality measures.
    • Responsible for outreach telephone calls to patients who are due for preventative services.
    • Provide patients with appropriate references and educational information related to the recommended screening or gap we are attempting to close.
    • Interact with providers, office managers and staff personnel to effectively communicate status of patient appointments and any barriers encountered. This will include telephonic interactions and in-person meetings.
    • Develop outreach communication to providers and staff to help facilitate patients coming back into the office for follow-up by their provider.
    • Coordinate outlook schedules / invites for meetings with providers and office staff as assigned
    • Provide up to date information to providers and their staff of any changes in payor requirements to ensure goals are met.
    Requirements


    • MA Certification or similar experience
    • 1 year of EMR experience required
    • Minimum 1-3 years' experience in community health care or human service setting, community health education and/or outreach position required
    • Basic Excel skills required
    • Experience supporting patients in a Medical Office setting and in a remote environment (Preferred)
    • Ability to work productively, either independently or with other individuals
    • Highly organized with the ability to keep accurate notes and records
    • Able to effectively communicate with patients, providers, and family members to effectively support care gap resolution
    • Excellent oral and written communication skills
    • Demonstrate a positive attitude and respectful, professional customer service
    • Acknowledge patient's rights on confidentiality issues and follow HIPAA guidelines and regulations
    • Knowledge of health care practices, insurance and common, current preventive service recommendations
    • Familiarity with medical terminology, common medical abbreviations, and chart documentation using Microsoft Office Suite 365
    • Competence in the use of computers, email, texting, and proficient with the current versions of Microsoft Office Suite, especially EXCEL and other commonly used business software
    • Must have a positive, can-do attitude and be willing to learn new skills and take on new assignments enthusiastically
