Job summary
The primary purpose of your job position is to efficiently deliver care to residents utilizing the assessment process (i.e., assessment, planning, intervention, implementation, evaluation). Ensures resident safety and well-being through observation and assessment of resident and communication of resident needs and changes among clinical team. Provides counsel and education to the resident and family support system as necessary.
Ensures that all required documentation and communication is completed in the time frame(s) determined along with the appropriate follow up steps that are needed.
