    Mental Health Therapist – MFT or LPC (Licensed or Intern)

    The Mental Health Therapist is a vital part of a team that helps people with behavioral health issues in a primary care setting. Their role involves handling crisis situations, conducting individual and group therapy sessions, and providing counseling for various needs like pain management and diabetic control. They collaborate closely with healthcare providers to create tailored plans for patients dealing with complex medical issues.

    Key Responsibilities

    Patient Care

    • Screen, intervene, and refer patients as needed
    • Consult with medical and behavioral health teams
    • Facilitate screenings and follow-ups
    • Support patients waiting for care

    Patient Records and Data Collection

    • Maintain accurate patient records
    • Document patient care and interactions
    • Ensure confidentiality of patient information
    • Assist in data collection efforts

    Care Coordination and Integration

    • Collaborate with healthcare providers
    • Work with onsite behavioral health team
    • Participate in administrative meetings

    Clinical Quality Performance

    • Compile statistical data
    • Engage in quality performance programs
    • Maintain a safe patient care environment
    • Participate in professional development

    Other

    • Perform assigned duties
    • Comply with all relevant laws

    Desired Skills:

    § In-depth knowledge of integrated care models

    § Strong personal integrity and communication skills

    § Experience in a medical setting

    § Familiarity with electronic health records

    § Ability to connect with patients empathetically

    § Bi-lingual: Spanish proficiency is a plus

    § Accessibility and mobility for behavioral health practice
