    Position Summary

    The Social Work Manager role provides leadership, supervision, and support to the Social Work teams at both the Holland and Muskegon Campuses.

    This Manager is responsible to plan, coordinate, monitor and ensure the provision of high-quality services that are delivered in home, community, and day-center.

    These services include managing evolving social work case management functions and assessment that are fully integrated within a dynamic interdisciplinary team-based model of care.

    This position requires advanced social work knowledge and skills.

    Additionally, this position may supervise both licensed and non-licensed personnel within the Social Work department as well as coordinate interns and volunteers.

    Required Qualifications

    Must possess a Master of Social Work degree from a program accredited by the Council on Social Work Education with a full unrestricted license (LMSW).

    Minimum of one year of experience working with the frail elderly, or long-term care population.
    Minimum, two years social work experience working directly with individuals.
    Medical Social Work experience, Supervisory experience, and participation in an interdisciplinary team-based model of care preferred.
    Strong desire to work in a team-based model of care
    Excellent communication skills
    Organized and have excellent time management skills
    Proficient in Excel, Microsoft Office, Outlook, and electronic medical records
    Strong sense of professionalism, good critical thinking, creativity, flexibility, and excellent problem-solving skills
    High ethical standards
    Comfortable in self-directed daily work
    Job Functions

    Provides supervision and oversight for social work staff, across both sites, on a routine basis and offers regular performance-based feedback.

    Supports professional development and engagement of social work staff. Documents appropriately in employee files. Oversees social work continuing education and licensing requirement compliance.

    Demonstrates an understanding of the
    PACE model of care and the principals of Advance Care Planning; educates and advocates for participant centered care both in the social work department and to the interdisciplinary team.

    Develops departmental processes and workflows that proactively address participant psychosocial and behavioral health needs.

    Demonstrates proficiency in all aspects of social work case management including the ability to both operate the Electronic Health Record (EHR) and train direct reports in all aspects of clinical documentation and record management.

    In coordination with other Directors, develops (and holds direct reports accountable to) policies, procedures, and documentation standards that are consistent with CMS and SAA regulations.

    Coordinates with other managers and directors to ensure work done in the Social Work department is supportive of organizational goals and priorities.

    Manages complex clinical situations in an efficient manner and in accordance with established agency defined procedure, social work best practice, and LifeCircles mission and vision.

    Participates in organizational committees and work groups dedicated to process improvement.
    Consults with appropriate leaders about high-risk clinical situations and organizational development issues.
