    Manager - CCBHC Implementation - Toledo, United States - Harbor

    Description

    Harbor is looking for a Manager of CCBHC Implementation who provides ongoing coordination and oversight of the implementation of the Certified Community Behavioral Health Clinic (CCBHC) model across the agency. The CCBHC Implementation Manager will be responsible for the ongoing implementation of programmatic changes to meet CCBHC certification criteria, monitoring and reporting on required Clinical Quality Measures (CQMs), and assist with clinical supervision and agency training needs related to model fidelity. This individual is also responsible for running the population health management reports and will work with the team members as needed for analysis and stratification.

    Position is full-time

    Education/Experience/Other Requirements:

    • Master's degree and license in behavioral health required (LSW, LPC, LISW, LPCC)
    • Independent license in behavioral health preferred (LISW, LPCC)
    • Excellent leadership and communication skills required.
    • Able to work successfully with interdisciplinary teams.
    • Must have valid driver's license, acceptable driving record, and be deemed insurable under Harbor's vehicle insurance carrier.
    • CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.
    • Required to obtain Health Officer certification within 6 months of employment.
    Essential Job Competencies/Primary Duties:

    1. Exhibits leadership and team-building skills to promote a positive culture among the team, among the department, and across the agency.
    2. Assists in operationalizing and implementing the CCBHC model of care across the agency.
    3. Tracks data and outcomes as required for population health management, CCBHC Clinical Quality Measures (CQMs), and other assigned client/program measures.
    4. Demonstrates leadership and supervision skills that leads to acceptable/expected levels of productivity, efficiency and effectiveness of any assigned team members in expecting and providing excellent care to clients.
    5. Assists in initial and ongoing training for new clinical staff and agency-wide CCBHC trainings.
    6. Works in coordination with other clinical leaders and Physician Led Patient Centered Teams to ensure coverage of services, supervision, and crisis support.
    7. Provides personnel management functions for all assigned staff including supervision, annual evaluations, involvement in hiring, disciplinary actions, etc. Any disciplinary action involving time off without pay or termination requires coordination/consultation with supervisor and Human Resources.
    8. Assists in innovation, development and implementation and evaluation of services, including outcome and grant reporting
    9. Participates in community meetings and workgroups focused on enhancing services and community resources.
    10. Participates in staff meetings, serves on committees as appointed, and is actively involved in continuing education and professional organizations as appropriate.
