    Obtain patient history and document consultation details, as pertinent to palliative care.
    Facilitate potentially difficult conversations with patients, surrogates and care teams.
    Collaborate with other healthcare providers, including hospice, to facilitate goal-concordant patient care.


    Licensure/Certification/Registration:

    Nurse Practitioner credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date required.

    Registered Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date required.

    Master's degree of Nursing required.
