    SUMMARY:

    The Case Manager- CSP (Community Support Program) must have the ability to conduct patient interviews, complete basic assessments, and engage patients into treatment and continuing care planning This role requires knowledge of and experience with community resources and referral systems and/or the ability to work in collaboration with community based providers and agencies. The Case Manager must have basic knowledge of mental health and addiction recovery processes, services, and support.

    ESSENTIAL JOB FUNCTIONS/DUTIES/RESPONSIBILITIES:
    • Engage with patients and participate in case management service plans.
    • Assist patients to successfully connect patients to post-acute levels of care.
    • Interact with patients, families, and community stakeholders with sensitivity and professionalism.
    • Assist patients with obtaining benefits, housing, and medical appointments.
    • Develop crises plans to ensure that patients have access to services when needed.
    • If indicated, accompany (including transporting) patients to community resources and other services as dictated by their service plans.
    • Participate in staff supervision and in-service training programs.
    • Establish linkages and utilize community resources to develop effective recovery support systems.
    • Participate in team meetings to review patient progress. Communicate with referral sources and advocacy groups.
    • Maintain patient records in compliance with existing Gosnold standards. Enter data into electronic billing system to record billable services and external payers (MBHP, Beacon, etc.).
    • Track outcomes for selected patients as necessary to comply with contract requirements (e.g., UBH).
    • Promote and support recovery concepts, develop continuing care plans and facilitate referrals to continuing care placements.
    • Perform all other duties as assigned.
    Minimum Knowledge, Experience and Skills Required:

    The requirements listed below are representative of the knowledge, skills, and/or abilities required to perform the job.
    • Education: Bachelor's Degree in a counseling or related discipline required.
    • Experience: Minimum two (2) years of experience in working addiction field.
    • Skills: Ability to interact with addiction patients with sensitivity and in a non-judgmental manner.
    Certificates, Licenses, and Registrations: Valid clean Driver's License.
