    MDS Coordinator (Minimum Data Set Coordinator) - RN or LPN (Clinical Case Manager) is needed at The Heights of Summerlin to manage the RAI Process Assessments. We offer a great and rewarding career with our seasoned team of professionals. Local candidates only please.

    Skills, Abilities, and Responsibilities

    The RN or LPN MDS Coordinator - Clinical Case Manager will assist in the responsibility to capture Medicare and Managed Care residents care needs. This will include the oversight of all service provided during inpatient stay from admission to discharge. This position is also responsible for the accurate and timely completion of all OBRA required assessments. This role is viewed as a key financial and clinical member of the management team.

    Candidates must have a current nursing license, experience as an MDS Coordinator, and familiarity with the RAI process and Case Management

    RN or LPN MDS Nurse Coordinator - Clinical Case Manager Qualifications
    • Must have a current, active license to practice as a Registered Nurse (RN) in Nevada
    • Must be well-versed in inter-personal communications.
    • Must be familiar with the RAI process and Case Management
