  • Position Overview:

    The Caregiver is a member of the home care team. The caregiver is responsible for the basic daily care of the client. Caregivers will help maintain care records for the client and communicate with other members of the care team. Caregivers report to the Nurse Manager.

    Essential Functions:

    1) To enable patients to stay in their homes by monitoring and recording the client's physical and mental condition.

    2) To maintain a safe, secure and healthy client environment by following necessary standards and procedures.

    3) To provide client support and personal services.

    4) To protect the home health care company by adhering to professional standards, homecare policies and procedures, federal, state, and local requirements.

    5) To follow HIPAA guidelines regarding all client PHI.

    Responsibility of a Caregiver:

    1) Monitors client condition by observing physical and mental conditions, intake/output, and reports any changes to the Nurse Manager.

    2) Provides personal services such as bathing, dressing and grooming.

    3) Teaches clients how to use cane or walker, special utensils to eat, techniques and equipment for personal hygiene.

    4) Assist in planning and preparing meals and snacks according to specific dietary requirements as outlined in the client's care plan.

    a. If necessary, the Caregiver will help with client feedings.

    5) Observe conditions of the client's residence. Report on any changes to the following:

    a. Client's physical behavior

    b. Client's mental behavior

    c. Potentially unsafe environment changes to the supervising nurse.

    6) Enhances service reputation by accepting ownership for accomplishing new and different requests.

    7) Explains the necessary procedure to the client prior to performing it.

    8) Light housekeeping duties as needed.

    9) Maintains continuity of client care by documenting and communicating with other caregivers in the client's care plan (book) and in the system.

    10)Participate in in-services and training as required.

    Skills/Qualifications:

    Independence, flexibility, good judgment, strong verbal and written communication, physiological knowledge, nutrition, ability to maintain a good client relationship, listening skills, time management, creates a safe environment, health promotion and maintenance, nursing skills, client service oriented, and emotional control.

    Valid driver's license and reliable transportation.

    CNA/LNA license preferred, but not required
