Job summary
The Nurse Practitioner is responsible for conducting required hospice face-to-face encounters to determine and document patient eligibility for hospice recertification in accordance with CMS regulations.The NP works collaboratively with the hospice interdisciplinary team (IDT), hospice medical director, and attending physicians to ensure timely, accurate, and compliant evaluations that support high-quality end-of-life care.
Qualifications
- Current, unrestricted Nurse Practitioner license in the state of practice.
- National
NP certification (AANP or ANCC).
