A facility near Cresecnt City, California is looking for a full-time, permanent, direct-hire Home Health Physical Therapist to join their team. See details below. 

Home Health Physical Therapist Opportunity 

• $45.00/hour starting pay
  
• Monday–Friday, flexible scheduling, no weekends
  
• The Home health Physical Therapist can expect 2–6 visits daily
  
• Travel averages 50 miles, may extend to 100 miles
  
• Federal-rate mileage reimbursement
  
• WellSky EMR documentation
  

Home Health Physical Therapist Qualifications

• Licensed Physical Therapist in the state of practice with current certifications and credentials
  
• Experience in home health care or outpatient rehabilitation, with the ability to manage multiple patient visits per day
  
• Strong clinical, organizational, and communication skills, including proficiency with electronic medical records (WellSky EMR preferred)
