- Support clients and staff in the Benchmark crisis support homes.
- Coordinate short-term out-of-home placements with stakeholders.
- Develop discharge plans for discontinuation from crisis support services.
- Follow up with the individual's IDT to ensure adherence to recommended services.
- Provide information to the mobile crisis team.
- Quickly facilitate referrals to prevent crisis escalation.
- Manage record-keeping functions and integrate them with CIS system.
- Complete necessary documentation for reimbursement through primary funding sources.
- Develop and implement safety plans when needed.
- Report any suspected abuse, neglect, or exploitation to supervisors.
- Be flexible with schedules to provide necessary services and care.
- LCSW or LPC certification
- Valid driver's license and auto insurance
- Experience and competence in the profession with relevant licenses and certifications.

Benchmark is seeking a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to join their team in the crisis support homes for adults with IDD in Acworth, GA.
