    • Assesses individuals served using strengths-based perspective.
    • Determines eligibility for services and defines desired life outcomes utilizing trauma-informed practices.
    • Briefly follows up with interventions delivered at home or other locations such as residential settings, emergency facilities etc., without causing any disruption amongst clients.
