NorthKey Community Care is seeking a full-time Disability Care Case Manager to work with individuals with intellectual and developmental disabilities (IDD). The program provides services to adults with disabilities, assisting them with becoming more independent and involved in their community.
+Qualifications
* Bachelor's degree in social work, Human Services, or related field;
* Relevant knowledge, training, and experience in the behavioral health field;
* Good written and verbal communication skills;
* A valid driver's license and the ability to operate a motor vehicle.<+Responsibilities
- Engage clients in a structured collaborative relationship that empowers them to gain necessary skills in various areas related to recovery self-management of mental health substance abuse or developmental disability.<- Promote environmental psycho-social health maximize independence improve maintain lifelong wellbeing functional status.- Provide evidence-based care concerning complex cases of vulnerable individuals serious mental illness emotional disturbance chronic substance addiction or developmental intellectual disability.- Effectively address functional challenges of served by identifying addressing risks current potential gaps prevent unnecessary deterioration hospitalizations emergency room visits loss independence vocational academic social functioning.- Complete requests prior authorizations indicated provide necessary services.- Participate development individualized interdisciplinary plan care for needs are coordination.- Educate client families significant others resources assist identified needs- Adhere organizational policies procedures documentation training appropriate timely services clients.
