We provide comprehensive medical care and leadership in long-term acute care hospitals, acute rehabilitation hospitals, skilled nursing facilities, assisted living facilities, memory care facilities and senior living facilities that delivers improved access to care, efficient and cost effective services.
Oak Medical's professional team is made up of:
Internal medicine physicians
Psychiatry services
The Nurse Practitioner (NP) functions under the clinical supervision of licensed physicians, provides diagnostic and therapeutic health care to patients in Skilled Nursing and Assisted Living environments.
Provide psychiatric care to various and/or specific patient levels- adults/geriatric.Establish and document reasonable medical diagnoses.
Order pertinent diagnostic laboratory and radiology testing for the medical diagnosis or presenting symptom.
Create and implements a medical plan of care.
Perform diagnostic and/or therapeutic procedures within his/her level of training and expertise.
Perform timely documentation and billing.
